Job Description: The Public Health Nurse (PHN), as an integral member of the Maternal and Children's care network provides nursing services within the framework of Population Health Promotion and Primary Health Care. The Public Health Nurse translates knowledge from health and social sciences to support health-enhancing behaviors of individuals, families and population groups through targeted interventions, referrals, consultations and advocacy. Inherent in this role are knowledge, skills, and abilities specific to health promotion, prevention, and protection. This employee works relatively independently assessing wellness, counselling, teaching and identifying community health needs. Employees are expected to make decisions based on sound assessments in accordance with nursing policies and procedures.
