HOURS OF WORK:
The stated hours of work are approximate as schedules are subject to change with the operational needs of the Health Centre. Changes made to hours of work will follow the terms of the applicable collective agreement, company policy and/or legislation.

WHAT YOU’LL DO:

The Nurse Navigator (NN) Oncology will support patients navigating Cancer Care services. The NN will give awareness of specialized services supporting the social, psychological, emotional, spiritual, quality of life and functional needs of patients and their families. The role supports patients with complex needs in the survivorship phase of their care.

The NN is a specialized oncology nursing role focused on the safe, effective, and coordinated transition of patients who are in remission or cured of cancer from oncology services back to primary care providers.

NN will be the primary contact for patients/families and liaise with multidisciplinary team members. They will build formal relationships across existing system silos to ensure seamless transitions as patients graduate from acute care phase to survivorship phase. This role is integral to optimizing system capacity and ensuring appropriate utilization of oncology resources.

The NN functions as a clinical expert, program developer, educator, and system navigator. Working collaboratively with oncology teams, primary care providers, and community partners, the role leads the development, implementation, evaluation, and continuous evolution of survivorship care models aligned with best practices, Ontario Health (Cancer Care Ontario) standards, and emerging evidence.

WHAT YOU’LL BRING:

• Current Registration with College of Nurses of Ontario required (in good standing)
• Baccalaureate of Nursing or equivalent
• Five years of recent clinical nursing experience in administration and management of Systemic Chemotherapy and clinical assessment in Oncology
• Completion of Provincial Standardized Chemotherapy & Biotherapy Course
• Completion of Foundations in Oncology Nursing Practice Course
• BCLS provider certification
• Master’s degree in a health care associated field preferred
• Oncology certification (e.g., CNA Oncology Certification) preferred
• Additional education in program development, leadership, or quality improvement preferred
• Completion of the Patient Navigation course (de Souza Institute) preferred

WHO YOU ARE:

• Advanced knowledge of oncology care across the cancer continuum, including survivorship
• Demonstrated experience in program development, implementation, and evaluation
• Strong clinical assessment, critical thinking, and problem-solving skills
• Excellent communication, collaboration, and relationship-building skills
• Ability to work independently and in an interdisciplinary environment
• Strong organizational and project management skills
• Proficient computer skills (e.g., EMR systems, Microsoft Office)
• Demonstrated ability to engage stakeholders and lead change
• Experience in survivorship care models or transition programs
• Knowledge of primary care systems and community resources
• Experience in quality improvement and/or research initiatives
