Job Description

Position Summary

The Diabetes Patient Educator functions to assess treat and offer education for diabetes patients and patients families. This role requires sound clinical judgement and ability to function within a pre-determined set of medical directives. The diabetes educator practices in accordance with SJHH mission and values policies and procedures and the College of Nurses Standards and Guidelines.

Qualifications
- Bachelor of Science in Nursing (BScN)
- 2 years related experience
- Current registration with the College of Nurses of Ontario required
- Certified as a Diabetes Educator through the Canadian Diabetes Educator Certification Board
- Demonstrates commitment to the value of Service Responsible Stewardship Community Responsiveness and Respect.
- Strong verbal and written communication skills.
- Experience in teaching patients in classrooms

Responsibilities
- Utilizes the nursing process assesses plans implements evaluates and documents the delivery and management of nursing care for individuals families and communities.
- Employs advanced nursing knowledge in clinical practice decision making critical thinking leadership research utilization and resource management.
- Demonstrates great depth and breadth of knowledge in diabetes care and is competent to lead and provide education for those situations which are more complex unpredictable and unstable.
- Applies critical thinking to integrate knowledge and technical skills in the provision of care to diverse patient populations as assigned
- Understands role expectations and seeks clarification when necessary; consults with physicians and other health care professionals when faced with situations beyond their knowledge skill and judgement; communicates effectively when collaborating and consulting.
- Coordinates and evaluates patient-centered care and discharge planning within the interdisciplinary team
- Provides patient centered goal directed consultative care for complex patients in various settings including in-patient outpatient and outreach locations. Supports self-management in diabetes care.
- Assesses patient needs by conducting a comprehensive assessment of previously and newly diagnosed diabetes.
- Develops an integrated plan of care in collaboration with the patient with diabetes and members of the health care team
